Flu Shots
Flu shots will be available starting Monday, October 7th for all employees and dependents who are eligible to access the Wellness Center. For your convenience, Cascade Health will be providing scheduled flu shots at the Springfield Wellness Center on Monday and Thursdays over a variety of time blocks. If you would like to schedule a flu shot, please click the schedule link below. (If you have a family member under the age of twelve you are scheduling for, please contact the clinic at 541-242-2816.)
Prior to your visit, please complete the consent form. Bring the completed consent form to your appointment time as this will reduce the amount of time spent in the clinic and improve patient flow. In addition, we also recommend for any patient receiving a flu shot to wear a short sleeve shirt.
