City Property & Equipment Loss
If you are an employee who needs to report damage or loss to City property, equipment, or City owned vehicle:
- Incident Analysis Form (IAF)
- Supplemental Vehicle Form – Complete if a City Vehicle has been damaged.
- Oregon Traffic Accident and Insurance Report – For drivers involved in an accident resulting in damage, towing, injury, or death.
- Supplemental Property/Equipment Loss Form – Complete if City property or equipment has been damaged.
